The Massage Therapist Earnings Questionnaire uses Google Forms. It does not request a name or email address and does not require respondents to sign in. It collects the answers a participant chooses to provide about work setting, location at the country/state/province level, appointment time, workload, compensation, and practice costs. Do not include client-identifying information, employer-confidential information, health information, account details, or a street address.
Questionnaire responses are available privately to the form owner at Silver Forge Technologies LLC and may be analyzed and summarized in educational content from The Practical Massage Therapist. Anonymous written responses are quoted only when the participant selects that permission; otherwise responses are summarized without direct quotation. The questionnaire is a voluntary audience sample, not a representative compensation study.
Google may process routine technical information associated with accessing and submitting the form under Google's own privacy terms. Although this questionnaire does not intentionally request direct identifiers, no internet service can promise absolute technical anonymity.
Information submitted through this site
Other than the questionnaire described above, the current website does not provide an account, contact form, payment form, or email subscription form. It does not intentionally collect personal information directly from visitors.
